  • Latest Military ID
  • Valid government issued ID or AMWSLAI ID
  • Original and Latest Payslip
  • Re-enlistment Order
  • Certificate of Re-enlistment (to be required if the Re-enlistment Order is less than six (6) months)
  • Provost Marshal Clearance or Certificate of Non-Pending Case
  • Certificate of Singleness or Spouse Letter of Consent and ID

Additional requirements for Buyout:

    • Certificate of Loan Balance or Latest Loan Balance Verification Slip
    • Official Receipt (must be marked “fully paid”) or Certificate of Full Payment
  • Latest Military ID
  • Valid government issued ID or AMWSLAI ID
  • Original and Latest Payslip
  • Re-enlistment Order
  • Certificate of Re-enlistment (to be required if the Re-enlistment Order is less than six (6) months)
  • Certificate of Non-Pending Case

    • For All Branch of Service except PA Civilian: 1 Payslip 
    • For PA Civilian: 1 Latest Payslip for Renewal Same Deduction, 3 Latest Payslips for new, additional or increase deduction
  • Appointment Order
  • Provost Marshall Clearance or Cert of Non-pending case or Discipline Law Order
  • Certification of Active Loan Validation Monitoring System (ALVAMS)
