MEMBERSHIP
REGULAR MEMBERSHIP
ELIGIBILITY
Military and permanent civilian personnel of the Philippine Air Force (PAF) assigned with the 410th MW and 420th SW and its component units, and the military and civilian personnel of 419th TSG prior to its dissolution.
BENEFITS
A regular member has the right to vote and to be voted upon as a member of the Board of Trustees. A regular member also has the privilege to avail and enjoy various savings, loan or welfare products and services of AMWSLAI.
HONORARY MEMBERSHIP
ELIGIBILITY
- Other organic personnel of the Armed Forces of the Philippines (AFP); the Department of National Defense (DND); the Philippine National Police (PNP) and the direct dependent/s of said other personnel.
- Dependents of the regular members
- Dependents of deceased members, whether regular or honorary who were of good credit standing during their membership.
- Regular employees of the Association.
BENEFITS
An honorary member has the privilege to avail and enjoy various savings, loan or welfare products and services of AMWSLAI.
MEMBERSHIP REQUIREMENTS
- Recent one (1) 2×2 picture in uniform
- Latest Military or PNP Identification Card. If ID is expired, submit Certification of ID renewal and present another valid government issues ID.
- One (1) Government issued ID
- Completely filled out Membership Information Form (MIF)
- Completely filled out Data Privacy Consent Form (DPCF)
- Recent one (1) 2×2 picture in uniform
- Training Unit ID or any government issued ID.
- Candidate soldier/trainee appointment order
- Photocopy of ID with three (3) specimen signatures
- Completely filled out Membership Information Form (MIF)
- Completely filled out Data Privacy Consent Form (DPCF)
- One (1) Recent 2×2 ID Picture
- Original copy of Birth Certificate (PSA Copy)
- Original Copy of Marriage Certificate (PSA Copy)
- 2 Government issued ID
- Photocopy of two (2) government issued ID with three (3) specimen signatures affixed
- Latest Proof of Billing (water bill, meralco, credit card)
- Completely filled out Membership Information Form (MIF)
- Completely filled out Data Privacy Consent Form (DPCF)
