Membership Inquiry Form

This online membership application does not replace the standard membership information form to be accomplished by an applicant. Applicants still need to accomplish a membership information form with specimen signatures and other needed documents to complete the AMWSLAI membership application process.  AMWSLAI shall validate the submitted information and our customer service shall contact applicants on the status of their application.
