A. Who are Qualified to Join:

1. AMWSLAI members who will update their Membership Information;
2. AMWSLAI members who will avail pension and salary loans during the promotion period.
3. Promo Period: 01 July 2021 to 10 June 2022
4. Raffle Draw: During the AMWSLAI 66th Anniversary

B. Prizes:

1. Grand Prize – 1 Toyota Vios
2. Consolation Prizes – Honda Wave (1 winner per Branch) – 21 winners

Note: A member may only win once. If a name is drawn more than once, he/she gets the
prize with higher value.

VIDEO/FILM ENTRY

A. Who are Qualified to Join:

1. Members with updated Membership Information Form (MIF) or Membership Update Form (MUF) may send:
   • 1  entry for himself/herself or  for other person/s
   • Entry may be individual or as a unit.

B. How to join:

Members have to fill up an online nomination form by accessing the online form and provide the following required information:

1. 1. Member Profile: Name, Branch of Service, Serial/Member Number, Email, Mobile Number
   2. Entry Video:
      • FB Profile where the video shall be posted
      • FB Video Link
   3. Answer to the Question: “Why should you win the “Bayaning Pilipino, Bayaning Serbisyo” Award?
   4. Testimonial Information (answer on the following questions):
      • How does AMWSLAI help you with your financial needs
      • Why do you choose AMWSLAI over others?

C. Entry of Information:

1. The entry must be in video format to be uploaded publicly to their personal Facebook Profile with the following hashtags: #AMWSLAI #bayaningpilipino #bayaningserbisyo #afp #pnp
2. Video links without the hashtags shall invalidate their award nomination entry.
3. The video content to be submitted must include the following:
   3.1 Opening Message: State member Name, Age, Rank, Branch of Service
   3.2 Your Story: Narrate a detailed story that allows heroism and selfless service. It  should be a compelling story of personal sacrifice and covers any of these virtues: Courage, Perseverance, Integrity, Loyalty, Teamwork, Respect, Leadership, Humility, Honor 
   3.3 Closing Message:
   • An inspiring message to our front liners, especially to fellow Military and Police. A unique and original quote is recommended to be used for voting and judging.
   • Video entry must be 2 minutes (minimum time) to a maximum of 10 minutes.
   • The video can be shot using a smartphone or any video capture device.

D. Criteria for Judging:

1. Popularity: 40%
   • AMWSLAI Facebook and Youtube pages –  1 view = 1 point
   • Number of likes: AMWSLAI Facebook and Youtube Pages – 1 like = 2 points
   • Number of shares: AMWSLAI Facebook Page – 1 share = 5 points                             
2. Production: 60%
   • Production Value – 10%
   • Entertainment Value – 20%
   • Relevance of the theme – 30%

E. Deadline of Submission of Entry:  31 May 2022

F. Selection and Announcement of Winners:

1. The award entries shall be judged during the 2022 AMWSLAI Anniversary.
2. Winners shall be announced on AMWSLAI website and social media accounts.

F. Prizes:

1. Grand Prize – ₱50,000.00 – 1 winner
2. Consolation Prizes – ₱10,000.00 – 1 winner per batch – 21 winners
